Closes [SS-520](https://linear.app/materializeinc/issue/SS-520) Docs for the `EXCLUDE ALL NOT NULL CONSTRAINTS` option on MySQL source tables, added in #39873. MySQL counterpart of #39842. Same structure and wording, minus the `UNIQUE NULLS NOT DISTINCT` note, which MySQL does not have, and with the MySQL constraint naming (`'PRIMARY'`) in the combined example. Also drops the now-shipped `warn-if-unreleased "v26.46"` tag on the constraint drop section, as #39842 did for Postgres.
